Millennium Physician Group is seeking a Nurse Practitioner, General / Primary Care to provide comprehensive primary care across the lifespan in MPG's ambulatory practice. You will evaluate, diagnose, and manage acute illnesses, chronic diseases, and preventive care in collaboration with physicians and interdisciplinary teams.

The role applies Master's-level clinical training within the APP scope, performing evidence-based assessments, ordering tests, immunizations, and health screenings, and

Qualificações

• Master's-level clinical training within the APP scope.
• Ability to evaluate, diagnose, and manage acute and chronic conditions.
• Experience with preventive care, immunizations, and chronic disease management.

Responsabilidades

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive care.
• Develop individualized treatment plans including medication management and immunizations.
• Perform office-based procedures and interpret tests within scope.
• Coordinate referrals and collaborate with care teams.
• Document encounters in the EHR and support value-based care initiatives.
