Qualifications

• Graduate of an Accredited NP or PA program.
• Active, unrestricted Massachusetts NP/PA license.
• National Board Certification.
• Mass DPH CS Registration active or obtainable post‑hire.
• DEA licensure/prescriptive authority active or obtainable post‑hire.
• Maintains BLS certification.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.
• Examinations, diagnoses and treatment in a primary care setting, including care plan supervision and referrals.
• Oversees monitoring and management of acute and chronic illnesses; author/supervisor of care plans.
• Partners with patients and families to manage care plans.
• Provides consultation regarding hospitalized patients in the panel.
• Provides direction and support to staff to assure departmental effectiveness and efficiency.
