Job summary

Atrius Health in Peabody, MA is seeking a Nurse Practitioner or Physician Assistant to join our APC team. You will provide comprehensive, longitudinal care, coordinate services, and supervise care plans for a defined patient panel in a primary care setting.

The role requires Massachusetts licensure as an NP/PA, board certification, and BLS. Experience with chronic disease management is preferred. Competitive compensation and a full benefits package are offered.

Qualifications

• Graduate of an Accredited Nurse Practitioner Program (Masters level preferred).
• Licensed by the Commonwealth of Massachusetts board of registration in Nursing.
• National Board Certification.
• Current active Mass DPH Controlled Substances Registration or ability to obtain post-hire.
• Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification, professional certification as appropriate.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of a specific panel of patients.
• Examination, diagnoses and treatment in a primary care setting with care plan supervision.
• Oversees monitoring and management of patients with acute and chronic illnesses; adjusts treatment regimens.
• Partners with patients and families to manage care plans.
• Provides consultation regarding hospitalized patients in the panel.
• Provides direction and support to staff to assure departmental effectiveness and efficiency.
