Job summary

Atrius Health in Quincy, MA is seeking an Advanced Practice Clinician (NP or PA) to join our primary care team. You will provide comprehensive patient care, coordinate care plans, and collaborate with physicians and staff to deliver high-quality care in a busy clinic.

You will oversee care for a defined patient panel, manage acute and chronic illnesses, and work with families to support ongoing treatment plans. Strong communication and team-focused approach required.

Qualifications

• Graduate of an Accredited Nurse Practitioner or Physician Assistant Program.
• Active, unrestricted Massachusetts NP or PA license.
• National Board Certification.
• Massachusetts DPH Controlled Substances Registration or ability to obtain post-hire.
• DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification, professional certification as appropriate.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.
• Performs all activities related to examination, diagnosis and treatment of patients in a primary care setting, including physical assessment and treatment; supervision of care plan, including consultations, referrals and communications with specialists as appropriate.
• Oversees monitoring and management of patients with acute and chronic illnesses; acts as care plan author and supervisor; adjusts treatment regimens based on patient response.
• Partners with patients and families to manage care plan.
• Provides consultation regarding hospitalized patients in the panel.
• Provides direction and support to staff to assure departmental effectiveness and efficiency.
