Alignment Health in Los Angeles area is seeking an Advanced Practice Clinician to deliver in-home assessments and telehealth support within the Care Anywhere program. You will work with an interdisciplinary team to provide proactive, patient-centered care in the homes of our members.

The role emphasizes collaboration, care coordination, and improving health outcomes for seniors while maintaining a balanced hybrid schedule with field and remote days.

Qualifications

• Master's degree from an accredited NP or PA program.
• Active California NP or PA license.
• Active NP or PA board certification.
• Current NPI number, DEA registration, and BLS.

Responsabilités

• Conduct in-home assessments, medication reviews, and health screenings for Alignment members two days per week.
• Provide telehealth support remotely from home three days per week.
• Educate patients and caregivers by aligning care plans with individual health goals and needs.
• Coordinate care across multiple providers, including primary care physicians, specialists, and ancillary providers.
• Lead and collaborate with an interdisciplinary care team to develop individualized care plans.
