Job summary

Qualifications

• Requires active NP or PA license and RN license with Furnishing number for NP role.
• Minimum 1 year of clinical or home care experience preferred.

Responsibilities

• Conduct in-home assessments and health screenings for Alignment members.
• Review medications and provide patient education.
• Identify diagnoses to be addressed in care management and active medical management.
• Coordinate care with PCPs, specialists, and other providers; discuss care plans with Regional Medical Officer.
• Lead a clinical team including nurses, health coaches, and care coordinators.
