Job summary

Alignment Health in California is seeking an Advanced Practice Clinician to deliver home-based medical and social care through the CAW program. The role focuses on in-home assessments, medication review, and preventive care to prevent hospitalizations and disease progression.

You will work with a multidisciplinary team to design and implement individualized care plans. Required are a Master’s NP/PA degree, an active NP/PA license, and relevant clinical experience; BLS and licensure credentials

Qualifications

• Master’s degree from an NP/PA program.
• Active NP or PA license.
• BLS certification and government-issued licenses as required.

Responsibilities

• Conduct in-home assessments, review medications, and perform health screenings.
• Provide patient education by aligning care goals with appropriate interventions.
• Identify diagnoses for care management and active medical management.
• Collaborate with Regional Medical Officer and team to develop care plans.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead a clinical team including nurses, health coaches, social workers and coordinators.
