Job summary

Alignment Health is seeking an Advanced Practice Clinician for the Care Anywhere program to deliver in-home medical and social support to seniors and chronically ill patients in California.

You will perform in-home assessments, review medications, educate patients, and coordinate with PCPs and specialists to build the most effective care plans. A Master's degree and an active NP/PA license are required, with EMR proficiency and leadership of care teams expected.

Qualifications

• Master’s degree from an accredited NP or PA program.
• Active NP or PA license; RN license with furnishing number required; NP/PA board certification; NPI and DEA.
• Proficient in electronic medical records and clinical documentation.

Responsibilities

• Conduct in-home assessments and health screenings for Alignment members.
• Review medications and provide patient education to tailor care plans.
• Identify diagnoses for care management and medical management.
• Coordinate with Regional Medical Officer and team to develop care plans.
• Collaborate with PCPs, specialists, and ancillary providers to coordinate care.
• Lead clinical team including nurses, health coaches, social workers, and coordinators.
