Job summary

Clare Medical of New Jersey is seeking an Advanced Practice Nurse Practitioner to perform comprehensive patient assessments and exams, including wellness and sick visits. You will evaluate chronic diseases, order and interpret tests, and collaborate with other providers to optimize care.

The ideal candidate holds an NJ APN license with at least one year of clinical experience, a current DEA, and is comfortable traveling within the region.

Qualifications

• EMR proficiency and familiarity with electronic health records.
• Experience with the Athena EMR system is a plus.
• Willingness and ability to travel within the region.

Job description

PRIMARY RESPONSIBILITIES:

Initiate and complete detailed patient assessment & physical exams

Wellness exams and sick visits

Evaluate and manage common and acute chronic diseases

Order, interpret and evaluate diagnostic and lab results

Refer and collaborate with other providers

Prescribing pharmacological and non-pharmacological therapies

REQUIREMENTS:

Computer literate & EMR Proficiency; Experience with Athena a plus

Unrestricted APN license in the State of NJ with at least 1 year clinical experience

Current DEA/CDS

Willingness and ability to work independently

Personable, passionate and energetic

Ability to travel within the region

Previous experience with Medicare eligible patients a plus.

Bi-lingual a plus
