Job summary

Clare Medical of New Jersey seeks an Advanced Nurse Practitioner for a Visiting Physician Practice in Southern Ocean County. You will perform detailed assessments, conduct wellness and sick visits, manage chronic diseases, order and interpret labs, and collaborate with other providers.

Travel within the region is required, with ongoing physician support. The ideal candidate has a NJ APN license with at least 1 year of clinical experience, current DEA/CDS, and EMR proficiency, plus a personable,

Qualifications

• Unrestricted APN license in NJ with at least 1 year clinical experience.
• Current DEA/CDS license.
• Computer literacy with EMR proficiency.
• Ability to travel within the region.
• Bi-lingual abilities are a plus.

PRIMARY RESPONSIBILITIES:

• Initiate and complete detailed patient assessment & physical exams
• Wellness exams and sick visits
• Evaluate and manage common and acute chronic diseases
• Order, interpret and evaluate diagnostic and lab results
• Refer and collaborate with other providers
• Prescribing pharmacological and non-pharmacological therapies

REQUIREMENTS:

• Computer literate & EMR Proficiency;
• Unrestricted APN license in the State of NJ with at least 1 year clinical experience
• Current DEA/CDS
• Willingness and ability to work independently
• Personable, passionate and energetic
• Ability to travel within the region
• Previous experience with Medicare eligible patients a plus.
• Bi-lingual a plus
