Job summary

Clare Medical of New Jersey is seeking an Advanced Nurse Practitioner for Visiting Physician Service. This role offers a part-time option (2 days a week) or a full-time schedule (Mon-Fri). Must maintain NJ APN licensure and DEA/CDS, with ability to travel regionally to deliver comprehensive patient care.

The position includes collaboration with clinicians, preventive and acute care, and opportunities for professional growth, with a supportive environment and travel reimbursement.

Qualifications

• Active APN license in NJ with at least 1 year of clinical experience.
• Current DEA/CDS registration.
• Willingness to travel within the region.
• Bi-lingual a plus.

PRIMARY RESPONSIBILITIES:

• Initiate and complete detailed patient assessment & physical exams
• Wellness exams and sick visits
• Evaluate and manage common and acute chronic diseases
• Order, interpret and evaluate diagnostic and lab results
• Refer and collaborate with other providers
• Prescribing pharmacological and non-pharmacological therapies

REQUIREMENTS:

• Computer literate & EMR Proficiency; Experience with Athena a plus
• Unrestricted APN license in the State of NJ with at least 1 year clinical experience
• Current DEA/CDS
• Willingness and ability to work independently
• Personable, passionate and energetic
• Ability to travel within the region
• Previous experience with Medicare eligible patients a plus.
• Bi-lingual a plus
