Responsibilities

• Maintain effective communication with the patient, interdisciplinary team, VA centers, and external agencies.
• Communicate with non-VA facilities and process secure messages.
• Notify patients of normal lab results.
• Develop and manage follow-up care tracking (consults, tests, etc.).
• Participate in team huddles to plan patient care and priorities.
• Enter information into the electronic record and manage priority tasks.
