Job summary

HarmonyCares is hiring a Nurse Practitioner to provide in-home primary care to patients with complex healthcare needs, focusing on personalized, patient-centered care. The role emphasizes thorough assessment and time spent with patients in home visits, addressing complex health issues in a non-volume driven manner.

The NP must be licensed in the state of practice, hold a DEA license, and possess or be pursuing board certification with CPR/BLS, and geriatric experience preferred.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Valid and unencumbered driver's license with good driving record.

• The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care.
