HarmonyCares is seeking a Nurse Practitioner to deliver in-home primary care for high-acuity patients. You’ll practice medicine with a patient-centered focus, in a team-based model, across markets where available.

The role emphasizes thorough patient assessment, care coordination, and ongoing management in patients’ homes, Monday through Friday, with support from clinical staff and, in select markets, a company car and driver.

Competenze

• Licensed NP in the state of practice.
• Federal DEA license or eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification.
• Valid and unencumbered driver’s license with good driving record.

Mansioni

• Provide primary care to high-acuity patients in their home, Monday–Friday.
• Practice medicine with a patient-care focus, not volume.
• Coordinate with support staff to address patient needs and ensure quality care.

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid and unencumbered driver's license with good driving record
