Qualifications

• Bachelor's degree in social, behavioral, or human services; prefer degree in social work.
• Must qualify as a QMHP-CS under Medicaid guidelines.
• Experience in employment, housing, budgeting or community resources; ability to communicate.
• Must have a Texas driver's license and insure a personal vehicle for Center duties.
• Eligible for a one year step increase based on performance.

Responsibilities

• Provide rehab case management to adults within Levels of Care 1, 2 and 3.
• Link consumers to medical, social, educational, and community resources; coordinate services.
• Develop and monitor Recovery Plans based on ANSA calculations and Authorized Level of Care.
• Perform ongoing assessments of living environment and document findings.
• Maintain monthly performance targets and work with consumers, families, and providers.
