Qualifications

• Bachelor's degree in social, behavioral, or human services; social work preferred.
• Must qualify as a QMHP-CS under Medicaid guidelines.

Responsibilities

• Provide Rehab Case Management for adults within Levels of Care 1–3.
• Linkage to medical, social, educational, and other support services.
