Job summary

• The Case Manager must hold a Bachelor’s degree in Human Services.
• A strong understanding of mental health diagnoses and treatment is required.
• Ability to work independently and collaboratively in a fast-paced recovery-oriented environment.

Responsibilities

• Perform psychosocial assessments and develop Person-Centered Recovery Plans.
• Connect adults and families with community resources and rehabilitative services.
• Document services in the electronic health record and coordinate crisis intervention when needed.
• Prioritize needs during crisis situations and participate in meetings and trainings.
