Qualifications

Responsibilities

• Assess patient condition and coordinate nursing care across disciplines.
• Develop and implement an individualized care plan with teaching and discharge planning.
• Administer direct care and medications as directed.
• Maintain infection control and a safe environment.
• Document care in medical records and communicate changes to RN/Clinician.
• Contribute to quality assurance activities within the unit or hospital.
