Qualifications

• PALS, BLS, TNCC preferred.

Responsibilities

• Assess patient condition and coordinate care with health disciplines involved in the treatment effort.
• Develop a written individualized care plan including teaching needs and discharge planning.
• Administer direct care when professional nursing skills are needed; administer medications.
• Practice infection control and maintain a clean and safe environment.
• Communicate changes in patient condition to Charge RN/Clinician.
• Develops and maintains current written plan of care; set priorities and goals with patient/family.
• Document teaching in the medical record and ensure alignment with physician instructions.
• Collaborate with staff and hospital committees to contribute to quality improvement at Stony Brook Medicine.
