Job summary

Community LIFE is seeking a Day Center Supervisor to oversee staff, coordinate daily tasks, and ensure Plan of Care adherence for frail elders. You will manage performance, facilitate interdepartmental communication, and promote a safe, welcoming center environment.

The role requires an associates degree or equivalent and two years in geriatric care with supervision. A PA driver’s license is required; CNA preferred.

Qualifications

• Associates degree or equivalent in public health, nursing or social work, or equivalent education/experience.
• Two years of demonstrated experience in a PACE model with at least one year in a supervisory capacity.
• Valid PA Driver's License; CNA preferred.

Responsibilities

• Supervise and train day center staff, assign daily tasks, and ensure Plan of Care adherence.
• Manage performance, communications with departments, problem solving and policy compliance.
• Maintain safe, pleasant environment for participants and oversee day-to-day operations.
• Apply knowledge to assess, plan care for, and serve frail elders per Community LIFE standards.

Job description

Summary of Job Duties:

The Day Center Supervisor is responsible for the general supervision and training of the day center staff (Aides) including, but not limited to: coordination/delegation of daily tasks to ensure the day center Plan of Care is followed, performance management, communication with other departments, problem solving and compliance with Community LIFE policies, procedures and regulatory requirements. The Day Center Supervisor maintains the day-to-day operations of the day center to ensure a safe and pleasant environment for participants. The Day Center Supervisor must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elders according to assigned responsibilities and Community LIFE standards.

Required Education:

Associates degree or equivalent in public health, nursing or social work or an equivalent combination of education and experience. An equivalent substitution for an Associates degree is a minimum of two years of demonstrated successful experience as an effective team member in a PACE model with one of those years in a supervisory or lead capacity.

Required Experience:

A minimum of two years’ experience in geriatric health care setting. Supervisory/lead experience is required.

Required Certifications and Licensure:

Valid PA Driver's License. CNA preferred.

• Ability to work effectively as a team member with staff, participants, providers and referral sources.
