Schedule: 7:30a-4p M-F with on call rotation + as needed based on participant care
Summary of Job Duties: The Home Care Supervisor (HCS) will provide consistent supervision to Community Care Aides using effective communication and guidance. This person will strengthen the relationship between the Home Care Department and the Inter Disciplinary Team (IDT). This person will provide hands on leadership in all aspects of home care. The HCS will develop partnerships with families and caregivers. The HCS may be required to make in-home visits as necessary for assessment and on-going case management. This position will participate in weekend/on-call rotation.
Benefits:Required Education: Graduate of one of the following:
Required Experience: 5 years of direct care experience with at least 1 year of experience working with the frail and elderly population. 2 years of supervisory experience. Experience teaching patients and other health care workers preferred.
Required Certifications/Licensure: Valid Pennsylvania COTA/L, PTA, CNA or LPN license. Valid PA Driver’s License
