Hilltop Community Resources seeks an ADRC Navigator to provide case management, option counseling, and assist with financial applications under the ADRC program. The role supports elderly and disabled adults in accessing long-term care services, performing outreach, and collaborating with community agencies.

The position requires managing a caseload, conducting assessments, developing service plans, and coordinating with partners to ensure service delivery.

Qualifications

• Bachelor’s degree in behavioral science, human services, or related field or four years of relevant case management experience.
• Equivalent combination of education and experience may be considered.

Responsabilités

• Manage a caseload of 30 clients and provide intake/referral services.
• Interview clients and families to assess long-term care needs and evaluate health and living situations.
• Develop a service plan based on evaluation of client needs and available providers.
• Assist with filling out applications for programs and follow up.
• Maintain accurate records in the State’s database (SUDS) and manage related documents.
