Job summary

Rehabilitation Center of Belmond is seeking an ADON/MDS Coordinator to join a supportive nursing team in a skilled nursing setting. You will lead care planning, ensure high standards of resident care, and collaborate with the nursing staff to optimize outcomes.

Ideal candidates hold an Iowa RN license or LPN certificate, graduate from an accredited nursing program, and have CPR certification. Previous MDS coordination and SNF CNA experience are valued; strong organizational skills are essential.

Qualifications

• MDS coordination experience is required.
• LPN SNF experience is preferred.
• CNA experience is a plus and may be considered in candidate evaluation.

Responsibilities

• Develop, monitor, audit, and modify each resident's care plan.
• Perform resident assessments and assist in discharge planning.

As an ADON/MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Rehabilitation Center of Belmond by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Iowa R.N. License or LPN certificate, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an LPN in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
