Job summary

Rehabilitation Center of Independence West Campus is seeking an experienced MDS Coordinator to join our caring skilled nursing team. You will develop, monitor, audit, and modify each resident’s care plan to meet individual needs and goals and assist with the discharge process.

To be eligible, applicants must hold an unencumbered Iowa RN license, be a graduate of an accredited nursing program, and CPR certification.

Qualifications

• Unencumbered State of Iowa RN License.
• Graduate of an accredited nursing program.
• CPR Certification.
• Prior experience as MDS Coordinator; RN in SNF for 1 year and CNA for 1 year preferable, but not required.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan to meet individual needs and goals, and assist in discharge planning.
• Provide leadership and coordination to ensure high-quality care for residents and regulatory compliance.

As MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Rehabilitation Center of Independence West Campus by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Iowa R.N. License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
