Rehabilitation Center of Belmond is seeking an ADON/MDS Coordinator to lead and support nursing staff, ensuring individualized care plans and high-quality resident care.

You will develop, monitor, and modify care plans, perform assessments, and assist with discharge planning to promote resident safety and comfort.

Qualifications

• Unencumbered Iowa RN license or LPN certificate.
• Graduate of an accredited nursing program.
• CPR Certification required.
• Experience as an MDS Coordinator preferred; willing to invest in the right candidate.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments and assist in discharge planning.
• Ensure high standards of care and safety for residents.
• Lead and support nursing staff as ADON/MDS Coordinator.

As an ADON/MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Rehabilitation Center of Belmond by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Iowa R.N. License or LPN certificate, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an LPN in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
