Job summary

Harmony Waterloo is seeking a skilled MDS Coordinator to join our caring team in Waterloo, IA. You will develop, monitor, audit, and modify each resident's care plan to meet individual needs and goals, while performing comprehensive resident assessments and supporting discharge planning.

Qualifications include Iowa RN license, accredited nursing program graduation, and CPR certification. Prior MDS Coordinator and SNF RN experience is preferred, with CNA experience valued.

Qualifications

• As a minimum, an unencumbered State of Iowa RN License, be a graduate of an accredited nursing program and CPR Certification.
• Prior experience as a MDS Coordinator and RN in a Skilled Nursing Facility is preferred.
• One year of experience as a CNA is preferable but not required.

Responsibilities

• Develop, monitor, audit, and modify each resident's care plan.
• Perform resident assessments and assist in the discharge process.

As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident's unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Harmony Waterloo by developing, monitoring, auditing, and modifying each resident's care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Iowa R.N. License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
