Rehabilitation Center of Belmond in Iowa is seeking an ADON/MDS Coordinator to join our caring, supportive team. You will lead nursing services, oversee care plans, and ensure high standards of resident care.

Qualifications

• RN or LPN license required; graduate of an accredited nursing program.
• CPR Certification required.
• Prior experience as MDS Coordinator preferred; 1 year as LPN in a Skilled Nursing Facility.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments and assist in the discharge process.
• Provide guidance to the care team to meet residents’ individual needs and goals.
