Job summary

AMFM Healthcare, through Mission Prep Healthcare, is seeking compassionate Community Coordinators to support adolescents at residential facilities in Loudoun County, VA. The role involves coordinating transport, assisting with care plans, and fostering a safe, inclusive environment.

The position offers a $21–$25 hourly rate with a 2:30pm–11:00pm shift pattern and immediate openings. A mission-driven team emphasizes growth, collaboration, and patient-centered care.

Qualifications

• Education: High School Diploma or GED; Bachelor's preferred.
• Background checks required including ADP Selective Screening Services and other Federal/State checks.
• Valid Driver's License with required driving history and insurance eligibility

Responsibilities

• Coordinates and transports clients to daily activities and ensures safety.
• Uses intervention and communication skills with clients and families daily.
• Adheres to Joint Commission policies and infection control procedures.
• Monitors medication management with the clinical team and documents in EHR.
• Maintains residence cleanliness and a safe, caring environment.
• Engages with clients to support wellness and recovery journeys.
• Documents client activities in BestNotes and reports behavior changes.
• Participates in a multidisciplinary care plan respecting client-centered approaches.
