Job summary

Imagine Lincoln (Adolescent Outpatient) in Lincoln, NE seeks a Care Coordinator to help clients and families secure resources, co-facilitate group sessions, and support educational needs. The role spans onsite facility work and community activities, reporting to counseling staff and the multidisciplinary team.

You will supervise clients, manage schedules, de-escalate when needed, and assist with treatment and discharge planning while maintaining confidentiality and professional boundaries.

Qualifications

• Bachelor’s Degree in relevant field or commensurate experience.
• 1+ year of relevant work experience required.
• Knowledge of English usage and excellent communication skills.

Responsibilities

• Interact routinely with clients, observe behaviors and communicate significant observations to clinical staff.
• Follow reporting and communication guidelines when interacting with staff, clients, and parents/guardians.
• Monitor progress on goals for assigned clients and adjust programming as needed.
• Ensure facility cleanliness and upkeep under leadership direction.
• Assist clients with scheduling community appointments.
• Assist clients with educational needs and collaborate with onsite educators and school resources.
• Provide facilitation and co-facilitation of approved group sessions.
• Oversee safe and efficient transport of clients to and from facilities if necessary.
