Qualifications

• Knowledge of co-occurring disorders and related needs.
• At least 21 years of age.
• Valid driver's license with acceptable DMV record (last 7 years)

Responsibilities

• Provide targeted support to individuals receiving services.
• Assist individuals and families to navigate community resources.
• Follow up with individuals during transitions of care to increase engagement in services.
• Ensure coordinated systems of care are responsive to assessed needs.
• Collaborate with teams to match services with individual goals.
