Northpoint Recovery Holdings, LLC’s Imagine Seattle (Adolescent outpatient) is seeking a Care Coordinator to help clients and families access resources, co-facilitate groups, and support educational needs in the adolescent program. This role involves supervising clients, maintaining programming schedules, and applying de-escalation skills as needed.

Qualifications

• Bachelor’s Degree in relevant field or commensurate experience.
• Must be at least twenty-one (21) years of age.
• 1+ year of relevant work experience required.
• Knowledge and skills in English usage and excellent communication skills.
• Ability to organize and prioritize competing priorities and work time.

Responsibilities

• Interact routinely with clients, observe behaviors and communicate observations to staff.
• Follow reporting and communication guidelines with staff, clients, and guardians.
• Monitor progress on goals and adjust programming as needed.
• Assist clients with scheduling community appointments and educational needs.
• Provide facilitation and co-facilitation of group sessions.
• Oversee safe transport of clients to and from facilities if necessary.
• Maintain confidentiality and adhere to policies and procedures.
