Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks an RN Clinical Manager to lead a Home Health team in Fort Wayne, IN. The role oversees patient-centered care, guides clinicians, and supports staff development in an in-office setting.

The ideal candidate will have home health and OASIS experience, RN licensure in good standing, and a strong clinical leadership background to ensure high-quality care outcomes.

Qualifications

• Bachelor’s Degree in Nursing preferred.
• RN license in good standing in state of practice and residence.
• Minimum two years clinical experience, plus ≥1 year in related management/supervisory role in home health care.
• Current CPR certification.
• Must meet agency pre-employment requirements as required by company/state regulations.
• Ability to use company documentation systems.
• Ability to communicate with all levels of personnel, verbally and in writing.
• Ability to work independently as well as part of a team.
• Capable of handling highly confidential information.

Responsibilities

• Conducts/participates in quarterly quality improvement meetings and oversees improvement plans.
• Collaborates with Branch Director on quality improvement activities and corrective actions.
• Oversees surveys/audits and KPIs related to quality of care with operations leadership.
• Completes documentation and chart reviews to identify variances and improved performance.
• Holds case conferences to ensure oversight of care and service coordination.
• Develops performance improvement action plans based on data and results.
• Ensures staff evaluations are fair and timely per company procedures.
