Job summary

You will document with OASIS, educate patients and caregivers, coordinate with physicians, and ensure smooth transitions into services. Valid Indiana RN license and at least 1 year of nursing experience required; driver's license and reliable

Qualifications

• Graduate of an accredited school of professional nursing.
• Current Registered Nurse license for Indiana (active).
• Minimum of 1 year nursing experience in hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license, reliable auto insurance, and personal transportation.
• Current TB test on file or within the required timeframe.

Responsibilities

• Complete patient admission visits and initial assessments for home health and hospice patients.
• Perform ongoing patient evaluations and adjust plans of care as needed.
• Develop and implement individualized plans of care with physicians and clinical team.
• Complete required clinical documentation, including OASIS assessments for eligible patients.
• Educate patients and caregivers on medications, diagnoses, treatment plans, and in-home care.
• Coordinate patient services and communicate changes to physicians and care team.
• Float across the Indianapolis area to provide admission coverage and nursing support.
