Job summary

Sierra Health and Wellness is seeking an Admissions Representative in Sacramento, CA to manage admissions across all programs and serve as the initial contact for potential clients and referring parties. You will enter data into databases, ensure accurate records, and work with staff to maximize referral success.

Responsibilities include tracking referrals, following up with clients, conducting wellness checks, processing payments, and maintaining professional relationships with referral

Qualifications

• High School diploma or GED required.
• Familiar with HIPAA/client confidentiality and personal rights.
• CPR/First Aid certification required.
• Current TB test and health screen.

Responsibilities

• Enter client data into facility databases; maintain accurate patient records.
• Track referrals daily and provide weekly/monthly updates to department director.
• Follow up with clients who have not connected to services.
• Conduct wellness checks at 30/60/90-day anniversaries of completion.
• Process patient payments and notify billing to ensure proper credits.
• Engage with referring parties to understand their needs and facilitate referrals.
