Chapters Health System seeks an Admission Registered Nurse to assess patients in living settings or hospital-based settings and initiate care into LifePath Hospice services in Temple Terrace, Florida. You will help ensure a seamless transition to Care Navigation and represent the organization with compassion.

The role requires an active Florida RN license, at least 1 year of nursing experience (hospice or hospital preferred), EMR proficiency, strong communication skills, and a valid driver’s

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; background in emergency department or ICU preferred.
• English communication required; Spanish bilingual preferred.
• Mobile driver; valid license and automobile insurance per policy.
• Reliable transportation to meet schedule.
• Ability to work in a team and manage emotional workload.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Responsabilités

• Completes clinical admissions for all Company services.
• Explains services to potential patients/families and addresses questions with empathy.
• Obtains physician orders and initiates Plan of Care.
• Initiates skilled nursing interventions to alleviate symptoms and maximize comfort.
• Obtains consents and documents per policy; communicates SBAR.
• Works with Care Navigation to ensure timely referrals and follow-up.
• Facilitates safe discharge planning and coordination with facilities.
