Job summary

Amedisys in Bloomington, IN seeks an Admissions Registered Nurse to join our home health team. This full-time daytime position includes weekend coverage and requires home health experience.

You will conduct initial assessments, plan and coordinate care, and support patients in healing at home. Ideal candidates hold an active RN license in Indiana, CPR certification, at least one year of RN clinical experience, and a valid driver's license.

Qualifications

• Current RN license, state-specific for the assigned work state.
• One year of clinical RN experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs start of care assessment, develops and implements the plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care per the plan of care and agency policies.
• Maintains continuity of patient care through communication with staff, physicians, family and resources.
• Initiates referrals when indicated.
• Promotes patient health through teaching and rehabilitative measures.
• Assists patients in learning self-care techniques.
• Other duties as assigned.
