Job summary

Amedisys admissions registered nurse role in Bloomington, IN, is a full-time position scheduled during days and weekends, focusing on home health admissions. You will assess, plan and implement care, perform OASIS assessments, and coordinate with patients, families, and care teams to ensure seamless transitions home.

We require an active RN license, CPR certification, and at least one year of RN experience.

Qualifications

Current RN license, state(s) you are assigned to work.
One year of clinical RN experience; if less than one year, approval required from VP of clinical.
Current CPR certification.
Valid driver's license, reliable transportation and liability insurance.

Responsibilities

Performs start of care comprehensive assessment, evaluation, development and implementation of plan of care.
Completes OASIS assessments timely and accurately.
Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
Helps maintain continuity of patient care by communicating with staff, physicians, family, and resources.
Initiates referrals when indicated.
Promotes patient health through teaching and rehabilitative measures.
Assists patients in learning self-care techniques.
Other duties as assigned.
