Select Home Health & Hospice in Indianapolis seeks an Admissions Registered Nurse to perform initial admissions and assessments for home health and hospice across the area. The role supports patient transitions and plans of care in collaboration with the care team.

Ideal candidates have an Indiana RN license, nursing experience, and ability to travel; strong patient education and documentation skills are essential.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license for Indiana.

Responsibilities

• Complete patient admission visits and initial assessments for home health and hospice patients.
• Develop and implement individualized plans of care in collaboration with physicians and the clinical team.
• Complete required clinical documentation, including OASIS assessments for eligible home health patients.
• Provide in-home nursing care and additional patient visits as needed based on agency needs.
• Educate patients and caregivers on medications, diagnoses, treatment plans and in-home care.
• Coordinate patient services and communicate changes in patient condition to physicians and the appropriate care team.
• Float throughout the Indianapolis area to provide admission coverage and additional nursing support as needed.
• Maintain compliance with applicable regulations, clinical standards and agency policies.
