Amedisys is seeking an Admissions Registered Nurse for full-time days in Bloomington, IN. The role offers home health visits with Monday–Friday work and a strong focus on patient-centered care in the home setting.

Responsibilities include comprehensive start of care assessments, timely OASIS completion, and delivering skilled nursing per plan of care. You will educate patients and coordinate referrals to support optimal outcomes.

Competenze

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as a RN.
• Current CPR certification.
• Valid driver's license, reliable transportation & liability insurance.

Mansioni

• Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.
