Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Admissions for a full-time position in Bound Brook, New Jersey. The Admissions RN will perform Start of Care visits, complete comprehensive initial assessments, and develop individualized plans of care for adult and geriatric clients.

Qualifications

• Current NJ RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.

Responsibilities

• Complete comprehensive OASIS Start of Care admissions visits for home health clients in the assigned territory.
• Perform thorough initial nursing assessments, including medication reconciliation and safety evaluations.
• Develop individualized plans of care in collaboration with physicians, clients, caregivers, and the interdisciplinary team.
• Identify client needs and coordinate nursing, therapy, social work, and other home health services.
• Review and execute physician orders to reflect client goals and needs.
• Educate clients and caregivers on diagnoses, treatments, and the home health plan of care.
• Communicate changes in condition to physicians and BAYADA team members.
• Ensure timely completion of OASIS assessments and admission documentation.
• Document assessments and care coordination in BAYADA EMR.
