Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for full-time home health admissions in Cranford, NJ. You will perform Start of Care assessments, establish plans of care, and coordinate across the interdisciplinary team to welcome new clients to BAYADA.

This role emphasizes autonomous fieldwork, clinical assessment, and strong client-family relationships across Monmouth, Middlesex, Union & Somerset Counties. NJ RN license and OASIS experience are required.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and manage a field-based schedule.

Responsibilities

• Perform comprehensive initial nursing assessments (OASIS SOC) for home health clients.
• Develop individualized plans of care in collaboration with the interdisciplinary team.
• Coordinate nursing, therapy, social work and other services.
• Educate clients and caregivers on diagnoses, medications, and home health plan of care.
• Document assessments and care coordination in the EMR.
• Support smooth transition from referral to admission and ongoing home health services.
