Citadel Healthcare in Rockford, IL, seeks an Admissions Coordinator to join our team. Under the Director of Admissions, you’ll manage admissions, gather patient information, and coordinate with referrals for a smooth transition of new residents.

You’ll work with staff, finance, and marketing to verify insurance, OBRA screening, and bed availability, ensuring accurate admissions information and timely follow-up.

Qualifikationen

• High school diploma or equivalent is required.
• Bachelor’s degree in a health, human services or related field is preferred.
• One year of related admissions or health services experience is preferred.

Aufgaben

• Respond to inquiries about admissions and eligibility.
• Gather patient information (medications, labs, history and physical).
• Coordinate with referrals and facility staff on admissions and bed availability.
• Communicate with finance on insurance verification and screening requirements.
• Assist with on-site assessments with the hospital liaison and DON as needed.
• Perform other duties related to admissions as assigned.
