Job summary

NHC Maury Regional Transitional Care Center is seeking an Admissions Nurse to help ensure smooth transitions for patients entering the center. You will work with referral sources, patients, families, nursing leadership, physicians, case managers, social workers, and the interdisciplinary team to review clinical information, coordinate admissions, and ensure safe, appropriate service.

Qualifications

• RN or LPN license in Tennessee (or compact license)
• Graduate of an approved nursing program
• Current CPR certification
• Experience in long-term care, skilled nursing, rehabilitation, hospital, admissions, or case-management preferred
• Strong clinical assessment and communication skills

Responsibilities

• Review clinical information and referral documentation for prospective admissions
• Coordinate admissions to ensure safe, timely placement
• Communicate with hospitals, physicians, case managers, social workers, patients, and families
• Collaborate with nursing leadership to coordinate admissions
• Review orders and documentation prior to admission
• Identify equipment, medications, or care needs before admission
• Maintain documentation of referrals, admissions decisions, and follow-up on missing information
