Job summary

Corewell Health is seeking an experienced RN Care Manager to support admission, stay and discharge planning for hospitalized patients at the Trenton site. You will integrate utilization management, care coordination and discharge planning to improve quality of care while controlling costs.

Responsibilities include caseload management, collaboration with clinicians, payers, and families, and ensuring timely transitions to appropriate post-acute settings.

Qualifications

• Bachelor's Degree Graduate of an accredited school of nursing.
• Will consider non-BSN RN if actively pursuing a Bachelors degree in nursing with completion within 2 years of hire.
• 2 years of relevant experience in the acute care setting.
• Three to five years’ experience in care management, utilization review, home care and/or discharge planning.
• Registered Nurse (RN) - State of Michigan Upon Hire required.
• Basic Life Support (BLS) - AHA American Heart Association preferred Or Basic Life Support (BLS) - ARC American Red Cross preferred.
• Case Manager, Certified (CCM) - CCMC Commission for Case Manager Certification Upon Hire preferred.

Responsibilities

• Identifies patients that need care management services (utilization review; care coordination; discharge/transition planning).
• Manage a caseload of patients, facilitating utilization management and care coordination during stay and coordinating discharge to appropriate level of care.
• Use critical thinking to determine alternative care paths and work on readmission initiatives to optimize patient flow.
• Review cases for medical necessity and obtain authorization for care and reimbursement with payers.
• Communicate with payers, patients/families, physicians and care team to ensure seamless transitions.
• Apply problem solving to discharge/transition planning and notify patients about coverage for hospital and post-acute services.

Education

Bachelor's Degree in Nursing

Registered Nurse license - Michigan

CCM Certification

BLS Certification (AHA or ARC)
