Job summary

City View Post Acute is seeking a compassionate Admission Nurse to coordinate nursing admissions and ensure a smooth transition into skilled nursing care. The RN will review records, assessments, medications, and orders, and communicate needs to the interdisciplinary team.

The ideal candidate is organized, detail-oriented, and able to support residents, families, physicians, and referral sources while complying with state and federal regulations and HIPAA.

Qualifications

• California RN license required.
• Prior skilled nursing or post-acute care experience preferred.
• Admission/clinical coordination experience is a plus.
• Strong nursing documentation and HIPAA knowledge.

Responsibilities

• Coordinate and complete nursing admissions for new residents.
• Review hospital records, referrals, diagnoses, medications, and orders prior to admission.
• Complete comprehensive nursing assessments and identify residents' clinical needs.
• Review and reconcile medications and relevant medical information.
• Communicate resident needs and clinical findings to the care team.
• Collaborate with physicians, nursing staff, case managers, therapists and other professionals.
• Ensure admission documentation is accurate and timely.
• Assist with developing and implementing individualized care plans.
• Educate residents and families about services and care plans.
