Job summary

PruittHealth Admissions Coordinator HCC coordinates admission, transfer and discharge of patients in accordance with federal, state and local guidelines. You verify financial information and complete admission paperwork with families.

You ensure rooms are ready, support patients and families during admission, and communicate details to other departments to ensure smooth transitions and ongoing care.

Qualifications

• High school diploma or equivalent; minimum two years college with courses in recreation, education, social work or clinical services.
• On-the-job training provided in admission guidelines.

Responsibilities

• Coordinates admission, transfer and discharge of patients.
• Verifies financial information prior to admission and completes admission paperwork with family/patient.
• Ensures patient room and related items are ready before admission.
• Maintains contact with patients and families to support during admission and transition.
• Visits new admissions to provide a smooth admission process and orient patients to the center.
• Communicates admission information to other departments to ensure coordinated care.
• Maintains up-to-date information about referral agencies and waiting lists.
