Job summary

NHC Maury Regional Transitional Care Center is seeking an Admissions Nurse to facilitate smooth transitions for patients entering the center. You will review clinical information, coordinate admissions with referral sources and the interdisciplinary team, and ensure accurate documentation prior to admission.

Ideal candidates are licensed RNs or LPNs in Tennessee with strong assessment and communication skills, and experience in long-term care or similar settings.

Qualifications

• Licensed RN or LPN in Tennessee; graduate of an approved nursing program; CPR certification required.

Responsibilities

• Review clinical information and referral documentation for prospective admissions.
• Determine if center can meet the clinical needs of referred patients.
• Communicate with hospitals, physicians, case managers, social workers, referral sources, patients and families regarding admissions.
• Coordinate admissions with nursing leadership and other departments to ensure safe and timely admission.
• Review diagnoses, medications, treatments, and orders prior to admission.
• Ensure admission documentation is complete and accurate.
