The role combines Admissions, Insurance Verification, Referral Management, and Healthcare Marketing to support hospital partnerships and community referrals. Strong communication and fast-paced healthcare experience are preferred.

Qualifikationen

• Experience in SNF admissions, healthcare marketing, case management, insurance verification, or post-acute care preferred.
• Knowledge of Medicare, Medicare Advantage, Medi-Cal, and managed care preferred.
• Strong communication, marketing, organization, and follow-up skills.
• Ability to develop relationships with hospital case managers and referral sources.
• Proactive, responsive, and comfortable working in a fast-paced healthcare environment.

Aufgaben

• Manage referrals from initial inquiry through admission.
• Review referrals through CarePort and other sources.
• Verify insurance eligibility, benefits, and authorization requirements.
• Coordinate referral review with Nursing, Business Office, and Administration.
• Maintain timely follow-up with hospitals, physicians, families, and insurers.
• Communicate acceptance decisions and coordinate admission arrangements.
• Build relationships with hospitals, case managers, physician groups, health plans, and referral partners.
• Conduct marketing outreach to generate new referrals and increase facility visibility.
• Track referrals, admissions, declines, and conversions.
• Collaborate with leadership to achieve census goals.
