Job summary

Avante at Ormond Beach is seeking an Internal Admissions Director to serve as the frontline for patients and families seeking placement in our skilled nursing facility for short-term rehab, respite, or long-term care. You will interface with hospital care navigators and nurse liaisons, guiding referrals through electronic systems and calls with physicians and case managers.

Qualifications

• Must possess the ability to communicate effectively, orally and in writing.
• Must deal tactfully with personnel, residents, visitors and the public.
• Must have knowledge of medical terminology.
• Must have knowledge of Medicare, Medicaid and other insurances.
• Must understand Federal and State regulations for SNFs related to admissions.
• Software knowledge - Outlook, Excel, Word
• Must be at least 18 years of age.

Responsibilities

• Frontline to patients and families seeking placement within our SNF for Short Term Rehab, Respite or Long Term Care.
• Interface with Care Navigators and Nurse Liaisons in hospitals.
• Communicate via electronic referral systems and calls with referral partners including Physicians and Hospital Case Managers.
• Represent the facility with area hospitals and medical professionals as part of the admissions process.
