Job summary

Avante at Ormond Beach seeks an Internal Admissions Director to be the frontline contact for patients and families seeking placement in our Skilled Nursing Facility. You will coordinate with Care Navigators and Nurse Liaisons and manage referrals to assess eligibility and explain benefits.

You will represent the facility with hospitals, physicians, case managers, hospice, and home health organizations, contributing to a team approach to resident placement and care transitions.

Qualifications

• Bachelor’s degree or equivalent experience in related field.
• 3-5 years’ experience in Long Term Care or similar healthcare organization.
• Knowledge of healthcare Federal and State regulatory standards.

Responsibilities

• Be the frontline contact to patients and families seeking placement in our Skilled Nursing Facility.
• Interface with Care Navigators and Nurse Liaisons within hospitals and with referral partners.
• Communicate via electronic referral systems and calls with physicians, case managers, hospice, home health and referral departments.
• Ensure benefits are acceptable and explained to the potential admissions; represent the facility with area hospitals and medical professionals.
